DATA PROTECTION AND CONSENT INFORMATION

   I acknowledge that I have been informed of the information under Article 13 of Regulation (EU) 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), provided by PFC CSKA EAD in connection with the issuance of a fan card allowing attendance at sporting events at the “Balgarska Armia” stadium, as well as of my rights as a data subject under the Regulation, and the contact details of the Data Protection Officer of PFC CSKA SAD: privacy@cska.bg, which form an integral part of this consent;

   I voluntarily provide my personal data.

   I acknowledge that the processing will be carried out by automated or other means, such as collection, recording, organization, structuring, storage, adaptation or alteration, retrieval, consultation, use, disclosure by transmission, in compliance with the principles of lawfulness, fairness, and transparency.

   I acknowledge that I have the right to obtain information about the personal data stored.

   I acknowledge that my personal data will be processed by the data controller specified herein, namely: PFC CSKA SAD.

   I acknowledge that I have the right to withdraw my consent for the processing of my personal data, partially or fully, at any time, by notifying the controller via email at privacy@cska.bg or at the address: 106 “Acad. Ivan Evstatiev Geshov” Blvd. I am aware of the voluntary nature of providing my data and the consequences of refusing to provide it.

   I acknowledge that I have the right to object and to lodge complaints regarding the processing and storage of personal data before the Commission for Personal Data Protection, which is the supervisory authority in the Republic of Bulgaria.
